Application Submission

Please download all of the following forms and fill them out completely.

  • Scholarship Application Form (Please see the below Supplemental Requirements)
  • Recommendation Form (two recommendations needed)

    Supplemental Requirements

    For all Northeastern Regional Chapter scholarship applications:

    1. Please submit, in addition to other required application material, your parents' tax return(s) and your tax return (if you filed one for last year). 
    2.   
    3. Please file your FAFSA application for the upcoming scholarship year and ensure that our Chapter receives your FAFSA result (Student Aid Report (SAR)) containing SAR's Expected Family Contribution (EFC) number – before our application deadline. Please note that the FAFSA application deadline may be different from this Chapter's scholarship application deadline. (For further information about FAFSA and FAFSA application, please visit http://www.fafsa.ed.gov).  
    4. Please explain (in the financial section of the application) how much you plan to earn during the upcoming summer and/or the academic year and how much of this you plan to use towards your educational expenses. If you cannot contribute to your financial needs, please explain why in the financial section of the application.  
    5. Please limit your essay to no more than 500 words.  
    6. All official school transcripts (starting from 1st year in college to the present day) must be sent to us directly from your school (with your school seal) before the deadline. If you are a high school student or will be a 1st year college student during the year in which you apply for our scholarship, you must have your high school send all your high school transcripts directly to us, and you must provide us with a copy of your SAT or ACT report (all before the deadline).  
    7. If you plan to attend (during the year for which you will be seeking scholarship from us) a school that is different from your current school, please submit a proof of admission from the new school.  
    8. Please indicate your school year (freshman, sophomore, etc.) during the year for which you will be seeking our scholarship.  
    9. Please provide us with your photo.  
    10. Please provide us with your cell phone number (or a secondary phone number/contact point) – so we can get in touch with you when needed.  
    11. Please have your recommendations sent directly to us.   
    12. Please enclose with your application, two self-addressed envelopes.
